How often should a tank be completely cleaned from top to toe? Should the gravel ever be removed completely and re-added with new gravel?

Hi, I normally spoon out 20% of the water once a week. Keep a light on above the tank during the day (its not near a window). The broad plant leaves are going a dark brown colour and the narrow leaf..
